THE ADHERENCE STATUS IN YOUNG ISCHEMIC HEART DISEASE PATIENTS AND ITS PREDICTOR

Ischemic heart disease (IHD) is the leading cause of cardiovascular-related premature death globally. However, young IHD patients are less likely to adhere to health recommendations. Thus, we aim to measure the adherence behaviour to the recalled health recommendations and related predictors. A cross-sectional study was conducted in a tertiary hospital from November 2021 to June 2022. Eligible young IHD patients were purposively sampled from outpatient cardiology clinics. They were evaluated using a validated self-administered questionnaire on sociodemographics, perceived severity of illness, health consciousness, eHealth literacy (eHEALS), and adherence to recalled health recommendations (MOSSAS). Patients with a MOSSAS score of < 5 are suboptimally adherent, while a score of ³ 5 is optimally adherent. Logistic regression modelling was used to determine the predictor towards suboptimal adherence behaviour. Of 136 participants, 73.5% were suboptimally adherent. Recall rates of health recommendations were highest for medication adherence (92.6% to 94.1%) and smoking cessation (94.1%), while the lowest was for regular exercise (53.7%). The least adhered recommendation was on socialising (20.0%), while reducing alcohol (72.1%) was the highest in those who scored ‘³ 5’. Only one predictor was identified through this study (type of IHD diagnosis). The odds of suboptimal adherence are higher (OR 3.765, 95% CI=1.227-11.551) in the necrotising type of IHD. Young IHD patients, especially those with necrotising type, have generally poor adherence to health recommendations. More focus and empowerment must be given to this group to prevent future graved complications.
