This study aimed to explore the relationship between self-concept and smoking behavior, as well as the underlying reasons for smoking among female university students. Mixed methods were adopted in this study using a sequential explanatory model, commencing with a cross-sectional survey. This was followed by a qualitative study to examine students' perceptions related to the results from the quantitative analysis. The survey included 217 female students from 15 faculties at Andalas University. Multiple logistic regression analysis was conducted with the variables of interest, such as smoking behavior, self-concept, level of knowledge, attitude, stress, and peer influence. The results showed that 13.4% of students smoked, 45.6% had a low self-concept, 40.6% had low knowledge levels, 35.5% had negative attitude, 39.2% experienced high stress, and 39.2% were influenced by peers. Furthermore, attitude was found to be the most dominant predictor of smoking behavior, with POR of 18.117 (95% CI 3.82-85.85). Qualitative data analysis shows that students do not act decisively when someone smokes nearby, and the presence of smokers influences attitude towards smoking. There was also a relationship between self-concept, level of knowledge, attitude, stress, and peer influence on smoking behavior. In conclusion, establishing Smoking Cessation Clinic (SCC) on campus was a preventative measure to address smoking behavior among female students.
